Outlook periodically asks a user for domain username and password

We recently upgraded from SBS 2003 with Exchange 2003 to Windows Server 2008 with Exchange 2010. I am having a problem with one client. The client is running Outlook 2003 SP2 under Windows XP SP3. Periodically (not always) when one user opens Outlook, it will be Offline, and when she tries to go Online, she is prompted for her domain username and password. This also sometimes happens when Outlook is open and she tries to open an email. The prompt is populating the username field with oldservername/username. I've tried to change it to newservername/username and then have her log in, but the dialog comes back with the old servername in the username field.

I also deleted the old Outlook profile and created a new one pointing to the new Exchange server. This fixed it for a week or so, but every time I get this working, the error eventually reappears.

As far as I know, this is the only user having this particular problem. When we first migrated everyone's document folders to a new file server, she also had issues with Word hanging when she tried to open documents. Got that fixed, but then this Outlook problem reared its head.

Many other users are getting Send-Receive errors relating to the Offline Address Book, and the OAB is not updating with the addresses of new domain users. So I'm wondering if the whole deal has to do with the OAB. I posted in the Exchange forums but it was suggested that this is more appropriate here. We are using MAPI to connect to the Exchange server.
