Using the Field Chooser

I am designing a form in Outlook and I understand that I have to bind a control to a field in order to store the data correctly. I have a significant number of Yes/No radio buttons on the form. To which control do I bind the Field to? e.g.

Has the form been submitted correctly   (rb)Yes   (rb)No

Anything I have placed on the form has a unique 'Name'
